Jakk Walsh (ABN 43 493 347 140), trading as Jakk Walsh OT, collects personal and health information to provide occupational therapy. This policy explains how that information is handled. It applies to jakkwalsh-ot.com.au and to services booked through it.
What we collect
Your name, and the name of the child, teenager or young adult
Contact details, NDIS number and plan-manager or billing details
Health, disability, school and functional information needed for assessment and therapy
Session notes, reports and correspondence
Booking and invoice records
We collect this from you, from a parent or guardian, and, with consent, from a school, support coordinator or other therapist.
Why we collect it
To provide occupational therapy, write reports, invoice the plan representative, meet professional record-keeping duties, and meet NDIS requirements. We do not use this information for marketing lists.
Who we share it with
Our bookkeeper, for invoicing
Your plan manager or the NDIA, when claiming NDIS funding
A school, support coordinator or other therapist, only with consent
Halaxy, which holds clinical and booking records
A regulator or another person, if the law requires it (including mandatory reporting)
We do not sell personal information.
Where it is stored
Records are kept in Halaxy and in secure practice files. Some providers used by Halaxy may store data outside Australia. We take reasonable steps to keep information secure. Health records are kept for at least 7 years after the last service, or until the client turns 25 if they were under 18, whichever is later.
Access and correction
You can ask to see or correct your information, or the information of a child you are responsible for, by emailing [your email]. We will respond within a reasonable time. If we refuse, we will tell you why.
Complaints
